As experts in the rational, therapeutic use of medications 1 , clinical pharmacists are uniquely suited to enhance palliative care services for adults and children through patient-specific medication considerations such as goals of care, care location, available routes of administration, pharmacokinetic and pharmacodynamic properties, and cost. This Fast Fact will review the role of clinical pharmacists within an interdisciplinary teams IDT with specific attention to hospice and palliative care teams.
A similar evolution of a clinical science took place when clinical pathology became established as a discipline separate from basic pathology. The progenitors of clinical pathology saw that a new type of specialist in pathology, one with firm roots in clinical medicine, was necessary if the study of pathology was to progress in providing improved patient care. Clinical pathology has been an established and recognized discipline for more than half a century. The growth and development of clinical pharmacology has proceeded much more slowly than that of clinical pathology. The earliest systematic clinical investigations into drugs were carried out by Harry Gold, MD, and his associates about 50 years ago.
As the relationship between pharmacist and physician developed, doctors began asking the pharmacist to go beyond offering advice and to initiate changes to the drug and dosage regimens in response to patients' signs and symptoms. Although hospitals had the legal flexibility to allow such practices, initially a physician was generally required to countersign each order. In time, hospitals approved blanket protocols that gave pharmacists direct authority to make these changes without prior or subsequent approval by a doctor. In the retail or community setting, however, pharmacists were not allowed to modify any regimen without a prescription by a physician. Prior to making a change, pharmacists were required to call the physician to ask for a new prescription. Today, as collaboration between pharmacists and physicians has grown and education and training of pharmacists has improved, a new model is being explored throughout the country: the community pharmacist as clinical provider of services in the community setting.
