Jacobson, PhD, James R. Social workers provide services to populations with mental health issues, including suicidal ideations and are thus at risk of experiencing a client suicide at some point during their career. Limited research using quantitative methods has explored the effect of client suicide on social workers, but little is known about their reactions from a qualitative standpoint—a method of study that is essential for fully understanding a social science phenomenon. This study addresses the gap in the literature by providing findings from an in-depth qualitative study of 25 mental health social workers who shared their experiences and reactions in the aftermath of a client suicide. Major themes were identified, including both professional and personal reactions of denial, anger, grief, and acceptance. Avoidance and intrusion, as well as additional themes of professional incompetence, responsibility, isolation, and justification were indicated and discussed in the conceptual framework of grief and secondary traumatic stress.
Thus, the very act of engagement in a therapeutic relationship presents a dilemma for the individual who is working hard to avoid past traumas. Likewise, the therapist may fall into common countertransferential traps of either confronting the trauma and loss material too quickly or colluding with the client to avoid painful content. In his first chapter, Muller provides a brief overview of attachment theory and related research, then shifting into a comprehensive discussion challenging common defensive structures used by avoidant individuals. Similarly, when Sandra experienced reliance or attachment-related emotions in the therapy, her strategy was to distance herself from the therapy, thereby creating a challenging dilemma in the treatment. As in the case of Sandra, many avoidant individuals may present in therapy as self reliant, independent and strong and then disappear when faced with traumatic material. In Chapter 2, Muller uses research to underscore a treatment approach which addresses the defensive patterns utilized by clients and outlines intervention strategies which actively addresses attachment-related experiences while challenging avoidance. Despite the challenges inherent in confronting avoidant clients on the vary material they seek to protect themselves from, Muller warns,.
